Please note that our facility is located in Florida, where temperatures can be high throughout the year. We recommend packing light, breathable clothing and any weather-appropriate items for your comfort. Each client is provided with a bedside tablet (powered by Volo) that includes access to reading materials, games, and entertainment. Clinical Director
With over 16 years of dedicated experience in the mental health field, Eric has made significant contributions across various settings, including inpatient psychiatric care, community-based mental health, corrections, and outpatient services. As a licensed mental health counselor in the State of Florida and a Qualified Supervisor, Eric brings a wealth of knowledge and expertise to his practice in mental health care. Throughout his career, Eric has demonstrated a strong commitment to leadership, having spent the last decade guiding and mentoring others in their professional
journeys. His passion for helping individuals navigate life’s challenges is evident in his compassionate and client-centered approach.
Outside of his professional life, Eric is a devoted father and husband who cherishes spending time with his family. He enjoys the outdoors, movies, music, and creating lasting memories with his loved ones.

Kristopher Polak
Intake Coordinator
Kristopher is the intake coordinator for Diamond Behavioral Health Tallahassee. He holds a bachelor’s degree in psychology from the University of Georgia as well as a master’s degree in clinical counseling from Lynn University. He got his start in the mental health and substance abuse field, working for nearly 7 years for a large and nationally respected organization as an admissions coordinator and manager.
Kristopher transitioned to facility-based operations soon thereafter, splitting his time between providing clinical counseling services such as individual and group therapy, and working as the Director of Admissions for a trauma-focused partial hospitalization (PHP) program. Kristopher joins Diamond Tallahassee with a wealth of knowledge consisting of providing exceptional client care, working closely with families, and an in-depth understanding of insurance and financials.
Kimberly O'Brien
Case Manager
With nearly a decade in the field of mental health and substance abuse, Kimberly O’Brien is a dedicated and compassionate leader in client care as a Case Manager. Originally from southeastern Massachusetts and then relocating to Florida, she has experience in various departments from operations to outreach.
Kimberly brings both passion and interdisciplinary skills to the team with a focus on the well-being of individuals. She is committed to ethical practices and cultural sensitivity, striving to create a supportive and inclusive environment for clients from diverse and complex backgrounds. Her interpersonal approach allows her to build trusting relationships with clients fostering an atmosphere of collaboration and mutual respect.
Kimberly is a SAMHSA-certified S.O.A.R specialist, playing a crucial role in assisting those with mental health and co-occurring disorders in navigating the Social Security Administration’s benefits system. She is adept at assessing client’s needs, identifying resources, and coordinating services throughout the treatment process to produce successful outcomes. She is a strong advocate in the community to break the stigma associated with mental health and a champion for the empowerment of individuals to lead fulfilling and healthy lives.
Frank Wolak
MD
Dr. Wolak is a dedicated psychiatrist with a passion for mental health advocacy and addiction treatment. He is board certified by the American Board of Psychiatry and Neurology and is an active member of the American Society of Addiction Medicine. He provides comprehensive care in a chemical dependency outpatient clinic, offering a range of addiction treatment services, including medication-assisted therapy.
A native of Rockland County, NY, Dr. Wolak earned dual bachelor’s degrees in biology and psychology with summa cum laude honors from the University at Albany, where he also minored in neuroscience. He was inducted into Phi Beta Kappa, the nation’s most prestigious academic honor society. He went on to earn his medical degree with honors from Saint George’s University in Grenada, West Indies. Dr. Wolak completed three years of his psychiatry residency at Bergen New Bridge Medical Center, one of the busiest psychiatric facilities in New Jersey, before finishing his training at Staten Island University Hospital. He currently serves as a clinical assistant professor for the Donald and Barbara Zucker School of Medicine at Hofstra/Northwell Health.
In his spare time, Dr. Wolak enjoys traveling, attending sporting events, spending time with friends and family, and snowboarding.

Through his personal experience, Adam refined his goal to establish a healing environment devoted solely to mental health. Operating independently under the umbrella of Diamond Recovery, Diamond Behavioral Health embodies Adam’s dedication to offering comprehensive and specialized mental health care.
At DBH, Adam spearheads a holistic approach to mental health treatment. It stands out as a specialized institution, distinctively refraining from addiction treatment to focus entirely on the myriad challenges posed by various mental health disorders. Sergio Muriel
COO
Sergio is an executive leader with over 20 years’ experience in the healthcare profession. He has his Masters in Education (MEd) from Florida Atlantic University (FAU) with a specialization in Mental Health and Rehabilitation Counseling. Sergio’s training and certifications include being a Licensed Mental Health Counselor (LMHC), Certified Addiction Professional (CAP), A.R.I.S.E. trained Interventionist, EMDR trained, as well as a Qualified Supervisor in the state of Florida, and an Internationally Certified Alcohol & Drug Counselor. His passion lies in helping individuals and families that have been affected by the disease of mental health and addiction.
Sergio has held several operational and administrative roles in his 20 year career and focus have been in mental health, substance use, and co-occurring disorders. Most recently, Sergio was the Chief Operating Officer for all of the Lifeskills South Florida psychiatric locations. As well as previous roles as Vice president of Clinical Services, and Executive Director for two additional leading healthcare organizations. Throughout his career, Sergio has received numerous recognitions, including the Sierra Tucson “Gratitude for Giving, Compassion Award” in 2016; “Above & Beyond” Award from Elements Behavioral Health in 2015; “Employee of the Year” from Miami-Dade County in 2007; “Appreciation Award”, Lindsey Hopkins Education Center-School Advisory Council in 2010; and “Appreciation Award”, Miami-Dade County, Mayor and Board of County Commissioners in 2009.
Sergio enjoys camping, vacationing, and having celebratory gatherings at his home with wife, two children, and extended family and friends. Sergio is also a devoted Miami Dolphins fan and season ticket holder and is still hopeful the Dolphins will make it to the Super Bowl once again!
Allison Trim
CMO
Allison Trim attended Temple University studying Public Health, which gave her a firm understanding of how to tackle societal problems systematically. Her education coupled with her personal experience in recovery affords her the expertise and compassion to truly help individuals struggling with mental health or substance use disorders.
Ms. Trim is also a certified interventionist (CNDAI II) with the ability to provide many resources for family members seeking help for their loved ones. Allison has worked in the behavioral health field for the last 5 years. She has played a pivotal role in assisting behavioral health facilities to grow and create processes and procedures to ensure that each patient is treated based on their individual needs. Ms. Trim has extensive experience in providing each patient and their family with the support they need to not just survive but to truly heal and thrive and maintain lasting recovery.

The typical length of stay at our facility is about 30 to 45 days. However, this can vary depending on each individual’s needs, progress, and treatment goals. We take a personalized approach, so some clients may require a shorter stay while others may benefit from an extended treatment period. Discharge planning is collaborative and begins early in the process to ensure continuity of care and successful transitions to the next level of support.
Do you offer co-occurring disorders treatment at your Tallahassee mental health facility?
We are dually licensed with DCF and ACHA; as such, we treat primary mental health and all co-occurring disorders, including, but not limited to: substance use disorders, psychotic disorders, thought disorders, psychotic disorders, and personality disorders.
Are mental health programs available for adolescents, young adults, or specific demographics?
DBH in Tallahassee only offers mental health programs for adults (18+). Young adults over 18 are provided with individualized care, as all clients are.
What are the room arrangements like at your mental health facility?
Semi-private Rooms include two full-sized beds, one private bathroom, one nightstand, and two dressers, on comfortable full-size beds.
What are the bathroom and shower arrangements like?
Clients will share a bathroom with one other client; showers are included in each bathroom.
Is the property friendly to those with mobility issues? If not, why?
Yes, our facility is in one building with ADA-compliant rooms, and all clients can participate fully in all programming.
Are there handicap-accessible rooms and bathrooms?
Our facility has 4 ADA-compliant rooms.
Is there television, cable, or WiFi access at Diamond Behavioral Health Tallassee?
TVs with Netflix in common areas; rooms provide a personal ipad per bed with access to movies, books, ipad games and puzzles that help with cognitive functioning, and self help books, such as AA or NA text books, and religious books to suit all religious affiliations.
What types of food are offered, and are dietary preferences or restrictions accommodated?
Our mental health center in Tallahassee offers your very own chef who offers a variety of dietary choices, including fresh eggs and fruit for breakfast, chicken, rice, steamed vegetables, pizza, taco nights, and many other options on a rotating schedule. We can also be accommodating for those with specific dietary needs. DBHT employs a full-time private chef.
How many individual and family therapy sessions do patients receive each week?
- Individual Therapy: 2 sessions per week with their assigned primary therapist, with session length of 45-50 minutes each. Frequency may be adjusted based on acuity and individual clinical needs.
- Family Therapy: 1 session per week (typically 60-90 minutes) when clinically appropriate and when family members are available and willing to participate. Additional family sessions may be scheduled during critical treatment junctures such as admission, treatment planning, and discharge preparation
